Student Enrollment Application

Student enrollment for the 2024-2025 school year is now open! Parents interested in enrolling their student(s) in our Academic Support and Enrichment Program must complete the student enrollment application form found below. Once you have submitted the student enrollment application, our program staff will contact the listed Family Leader to establish an appointment and complete the enrollment process. For more information about the Academic Support and Enrichment program and services provided to enrolled families, visit the What We Do page.​
